  1. Pour the whole milk and heavy cream into a medium saucepan. Add the cocoa powder, sugar and salt, and whisk until the cocoa is mostly dispersed.
  2. Set the saucepan over medium-low heat. Cook for about 5 to 7 minutes, whisking often, until the mixture is steaming and hot but not boiling.
  3. Reduce the heat to low. Add the chopped semisweet chocolate and whisk for 1 to 2 minutes, until completely melted and the hot chocolate is smooth.
